At Southwest Network, our goal is to not only provide behavioral health treatment for your child, but strengthen
your family. To do this, we will work with you to put together a Child and Family Team, or CFT,
which will make a plan to help you and your family.
The CFT will bring together the wisdom and support of family and friends as well as the resources, experience and
expertise of formal supports such as your case manager,
a family support partner, and other professionals. Together, the team will help your family and give you
the support you need to go forward.
They will meet as often as necessary, but at least once a month. They will also meet wherever you chooseit
could be your home or office, our office, the school, park, library, or any place that is convenient for you.
Your CFT will start the Child and Family Team process by letting you share your story, because you know your familys
situation best. This will help them get to know you and learn about your familys concerns, needs, hopes, dreams,
and strengths. Theyll also want to know about your familys culture and traditions to understand your unique way of life.
Your CFT will work with you to create goals that will help you get to where you want to be while making sure your
childs physical, educational, and mental health needs are met. These goals will be written in a plan that
spells out what services you need, as well as simple steps that can be taken to minimize the impact of a crisis.
Throughout the process, we will coordinate the right services to provide care thats individualized for your
child and will help rebuild and strengthen your family. Well also help you build a network of people and
community resources which will eventually replace us as your support system.
