SERVICES FOR ADULTS
Case Manager, Adult Services:
Case managers provide coordination of care for adults who are seriously mentally ill. This
intensive position requires compassion and attention to detail in order to assist each individual in
moving toward recovery and self-sufficiency.
ACT Team Case Management, Adult Services:
Assertive Community Treatment, or ACT, is a more intensive, evidence-based practice delivering
comprehensive treatment to persons diagnosed with serious and persistent mental illness. It
includes multi-disciplinary 24 hour availability in the community and provides intensive treatment,
rehabilitation and support services to people in their homes, in social settings and at their jobs.

SERVICES FOR CHILDREN
All members of our child & adolescent services team are expected to practice in a manner that is
consistent with the Arizona Vision and Principles:

service approaches when children or families are involved with multiple systems

Case Manager:
Case managers facilitate the strategic planning process for assigned children and their families.
As a member of the Child and Family Team, case managers are critical to the successful
partnership between the members of the support system. The minimum qualification for this role
is a Bachelor's degree. Preference is given to candidates who have a Bachelor's degree in
Sociology, Psychology, or other Social Science and/or work experience in behavioral health. We have
six locations from which our child & adolescent case managers serve the community. If this

Parent Partner:
Parent partners draw on their own experience to mentor and provide peer-to-peer support and
intervention to parents of children and adolescents who are receiving services from Southwest
Network. These professionals perform the role of assisting family members achieve stated goals,
deliver interventions/activities that build on strengths to achieve outcomes, and collaborate with
other individuals involved in the Child and Family Team. This position is field based and requires
travel throughout the Phoenix area.
