Overview of the Child and Family Team Process
The Child and Family Team process is not a program or a service. It is a
strategic planning process that involves the efforts of a family and
those who know them best, along with those who provide supports to them
in their home and community. The Child and Family Team (CFT) process is
based on the idea that families are most meaningfully and effectively
supported when their individualized service plans reflect their unique
preferences, traditions and goals for the future.
The core CFT includes the parents/guardian, the child who is receiving
behavioral health services, a case manager, family support partner and
therapist/clinical liaison. In addition to the core, the CFT can expand
to include the child’s siblings, extended family members, friends,
mentor, teacher and other professional service providers as the family
chooses. When more than one agency is involved with the family, they
work together in a collaborative way to help the family – and each other
– work toward common goals.
The CFT meets regularly to explore individual family strengths and
culture, identify the most pressing needs, develop strength-based
options, identify community resources to meet those needs and check on
progress toward meeting the goals. The CFT is designed around the 12
Principles to ensure family voice, ownership and access throughout the
process.
